Optimizing Life with Long-Term Conditions

If you are a person living with a long-term health condition, or a caregiver of someone with chronic illness, you are eligible to register for this free 6-week program. The Ontario Patient Self-Management Network announces.... First Time in Ontario! Master Training in Stanford’s Chronic Disease Self-Management Program. December 3,4,5,6,7 (Monday through Friday) 2007 in Toronto. Graduates of this training will be qualified to lead self-management workshops AND to train other workshop leaders. Please read the application form carefully before submitting. Application deadline: September 30, 2007.
